Long Term Care Program
Long term care encompasses a wide variety of living and care support arrangements including home health care, assisted living, group home living and skilled nursing units. Within this bevy of possibilities there are further distinctions, e.g. whether the facility or service is dementia specific.
This Long Term Care program is aimed at the entire range of families who are considering long term care, making the move or dealing with the challenges of using these care services. We'll spend time looking at the question of when to use long term care. In this section we’ll provide models for how to make the decision whether to use long term care and at what point a move should be contemplated. Next we'll work on the question of what and where –looking at the variety of long term care options available and providing information and tips on finding the right match for you and your family member.
The balance of the program will cover making the move to long term care and dealing with the special issues that can arise. Family care changes but does not end when a person moves to a facility. Managing the changes means learning new skills and understanding new rules. We'll cover how to work with staff, how to be an advocate for your family member's care and how to care for yourself in the meantime. For more information, call 612-624-9424.
